Neurosurgeons of "National Centre for Neurosurgery" JSC for the first time conducted a unique operation "Microsurgical removal of the left frontal and parietal lobe brain tumor using neuronavigation system under anesthesia retaining consciousness". It saved a 57-year-old man from West Kazakhstan region from disability, whose tumor was located close to the speech centre (Broca area) and motor area (precentral gyrus). The surgery lasted for two hours under local and intravenous anesthesia retaining consciousness. The problem of reduction and prevention of the neurological deficit after removing the mass lesions of the brain in functionally important areas is still relevant at the present time. It is very important not to damage the surrounding brain tissue and blood vessels while removing tumors in speech and motor area. Moreover, one of the adverse characteristics of gliomas is diffuse infiltration of the tumor into the surrounding brain parenchyma, which in some cases does not allow the total removal of the tumor. Total or maximum removal of the tumor demonstrates the greater survival with progression-free effect and predicts a significant improvement in overall survival comparing with a sub-total or partial excision of the tumor. Therefore, to maximize the removal of the tumor and define the boundaries of the tumor neurosurgeons used the neuronavigation system, and to prevent the damage of functionally important areas used neurophysiological monitoring during surgery and direct control over speech-motor centers in direct contact with the patient, which essentially determine the life quality of the patient. Physicians who participated to the operation: surgeon: S.K. Akshulakov, MD, assistants: N.A. Ryskeldiyev, MD, PhD, A.Zh. Doskaliyev, MD, PhD, G.I. Olenbay, neurophysiologist: V.K. Akhmetzhanov, speech therapist, aphasiologist: A.K. Sadyrbekova, neurologist, epileptologist: G.S. Ibatova, anesthesiologists: A.Z .Nurpeisov, PhD, M.A. Nurdinov.
