Successful removal of multiple cerebral aneurysms

Doctors from the Department of Vascular and Functional Neurosurgery performed a two-stage complex operation to remove multiple cerebral aneurysms.

According to a 74-year-old patient from Temirtau city, since the summer of 2023 the man had been bothered by severe headaches, lack of object vision and light perception, limited movements in the left eye, as well as drooping of the upper eyelid.

Doctors of the Department, based on MRI images and selective cerebral angiography, identified multiple cerebral aneurysms in the patient, an incidental giant fusiform aneurysm of the cavernous-paraclinoid segment of the left internal carotid artery, measuring 50.3 * 48.2 mm, an incident fusiform aneurysm of the paraclinoid segment of the right internal carotid arteries, measuring 16.5*14.5 mm.

Taking into account the presence of a giant aneurysm, the clinical and neurological status of the patient, and also there was an extremely high risk of aneurysm rupture and vessel occlusion during aneurysm embolization, the doctors of the Department at the consultation decided to perform the operation in 2 stages.

The first stage of the operation took place in November 2023. Doctors performed a protective extra-intracranial intervascular anastomosis between the frontal and parietal branches of the superficial temporal artery and the M3-M4 segments of the left middle cerebral artery. X-ray endovascular method of switching off the left internal carotid artery with microspirals. After the operation, the neurological status improved, and the patient's ptosis of the upper eyelids regressed.

A few days ago, the patient underwent the second stage of the operation - X-ray endovascular embolization of a fusiform aneurysm of the paraclinoid segment of the right internal carotid artery using telescopic installation of flow-redirecting stents.

As the patient himself notes, he feels much better and is satisfied with the outcome of the operation. “I would like to express my gratitude to all the doctors who operated on me. According to neurosurgeon Dr. Chingiz Nurimanov, in most cases the aneurysm does not manifest itself in any way, and a person can live for years without suspecting that he can die at any moment. “It happens that a person does not experience discomfort or alarming symptoms until the aneurysm ruptures. Therefore, in order to detect the disease in the early stages, it is necessary to perform MSCT – multislice computed tomography – at least once in a lifetime,” noted the neurosurgeon.

Surgery for a cerebral aneurysm is rightfully considered one of the most complex surgical interventions, requiring highly qualified specialists and modern medical equipment. The operation was performed on the patient for free within the framework of the compulsory medical insurance.
