List of documents for admission

Documents for admission to the retraining and advanced training cycles:

1. Copy of identity card;

2. A copy of the diploma of graduation;

3. A copy of the certificate of completion of the internship (for health workers in his absence: a document confirming his passage: an archival certificate or an entry in the work book); / clinical residency, residency; );

4. Copy of a specialist certificate;

5. For continuing education courses: a copy of the specialist’s certificate; upon changing the internship specialty - provide a certificate of retraining;

6. The order on the direction of training (in the direction of training with an organization);

7. A copy of the work book;

8. A copy of the medical book;

9. The permit (if available);


10. Travel certificate (if applicable);

11. A medical gown, surgical suit and replaceable leather shoes.
